Basic services include fillings, simple extractions, and anterior root canals — common repairs. Major services cover crowns, bridges, dentures, surgical extractions, and molar root canals — more complex procedures.
Purely cosmetic procedures like teeth whitening and veneers are not covered. However, restorative procedures that improve both function and appearance (like porcelain crowns) are covered under major services.
Yes. Your frames allowance and contact lens allowance are separate benefits. Use both in the same plan year.
