Residents of New York are insured through Renaissance Life & Health Insurance Company of New York (Binghamton, NY). Residents of all other states are insured through Renaissance Life & Health Insurance Company of America (Indianapolis, IN).
Members have the right to receive clear information about their benefits, file complaints and appeals, access covered services, and receive an explanation of benefits for each claim. For a full list of your rights under your plan, refer to your certificate of coverage or contact member support.
